A stay in our Transitional Care unit allows you to build confidence and self-care skills before going home after a hospitalization or rehabilitation stay. Oftentimes, Medicare or insurance coverage ends before a person feels prepared to return home. VMP Transitional Care offers a furnished studio apartment in order to provide you the opportunity to live independently while under the supervision of caregivers and licensed clinicians. Aside from the peace of mind this program brings, it also reduces the risk of hospital readmission and allows you time to further strengthen your body and manage your condition.
