24 Hour Care

It may be hard to imagine needing skilled nursing care, but there may come a time when you or a loved one needs 24-hour care. In addition to short-term needs for a period of respite or rehabilitation, VMP offers long-term care for people who can no longer care for themselves. We offer compassionate, high-quality healthcare - skilled medical and custodial care - 24 hours a day in a comfortable, safe, home-like environment.

Individual Care Plan

Our intent is to help each resident achieve the highest level of independence that is possible within our continuum care community. To meet this objective an interdisciplinary treatment team of a case manager, social worker, psychologist, occupational, speech and physical therapists, music and recreational therapists, chaplain, dietician and pharmacist work together to develop and maintain each individual's care plan. Our Chapel, medical clinic, gift shop, activity areas and hair salons are conveniently located for residents, all under one roof.

Ventilator Services

VMP Healthcare & Community Living offers ventilator care. Our highly specialized respiratory staff partners with the areas Pulmonary physicians to provide the best possible care for the patients we serve. These specialists all work together to meet the very needs of not only the patient, but their families as well. We promote weaning with a goal of sending patients back to their home. We also work with home care agencies to help make the transition home easier.

Dr. Hardgrove

VMP's Medical Director

As a geriatrician, Dr. Paula Hardgrove, MD, MPH, provides patient-centered care for acute and chronic, simple and complex conditions based on the unique needs of her patients. Her goal is to facilitate and maintain cognitive, physical and emotional functionality and quality of life that is aligned with her patients’ goals.

Dr. Hardgrove provides comprehensive geriatric assessments, capacity evaluations, goals of care/advanced care planning, polypharmacy management, complex care delivery and care for geriatric syndromes including dementia and delirium.

Dr. Hardgrove earned her medical degree at the University of Utah School of Medicine in Salt Lake City and completed her residency in Internal Medicine/Primary Care at Cambridge Health Alliance, Harvard Teaching Hospital in Cambridge, Massachusetts. She went on to complete a fellowship in Clinical and Advanced Geriatrics in Post-Acute & Long Term Care at the University of Wisconsin and VA Hospitals in Madison. Dr. Hardgrove holds a Master of Public Health (MPH) degree, served a Post-Acute and Long Term Care Medical Directorship and additional training in Gerontology.

We feel blessed to have Dr. Hardgrove as our medical director.
