SUMMARY
 
The Advance Practice Nurse (APN) is an experienced nurse who, acting within their professional boundaries, will provide care for the presenting resident from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care for urgent and non-emergent situations with the Skilled Nursing Home. The APN will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for residents within the long term care practice. The APN will demonstrate critical thinking in the clinical decision-making process. The APN will work collaboratively with the SNFists group and other attending physicians to meet the needs of residents, supporting the delivery of policy and procedures, and providing nurse leadership as required and is expected to perform hand off daily to the Nursing Leadership team and the SNFists group and other attending physicians.  
 
 
ESSENTIAL DUTIES AND RESPONSIBILITIES
Clinical Practice

  • Perform initial resident screenings and resident history and physical examinations.
  • Update resident records and check records for accuracy with each encounter.
  • Analyze test data and determine the need for follow-up and further treatment options.
  • The ANP will in collaboration with the MD evaluated lab results, order labs or other diagnostic tests.
  • Support residents to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
  • Provide education to the line staff as it relates to resident assessment, change of condition and appropriate documentation of findings.
  • Complete all analytics related to discharge back to acute care on a daily basis and provide summary report of findings on a monthly basis.
  • The ANP through a collaborative approach, will round throughout the skilled nursing facility during their entire shift and will assess and document findings on all residents with a potential or identified change of condition, new admissions, readmissions, new skin conditions or declining skin conditions and post all falls.
  • Clinically examine and assess resident needs from a physiological and psychological perspective, and plan clinical care accordingly.
  • Assess, diagnose, plan, implement and evaluate interventions/treatments for residents with complex needs. Proactively identify, diagnose and manage treatment plans for residents at risk of developing a long-term condition (as appropriate).
  • Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan .
  •  Prescribe and review medication for therapeutic effectiveness, appropriate to resident needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice. 
  • Work with residents in order to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
  • Prioritize health problems and intervene appropriately to assist the resident in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Use technology and appropriate software as an aid to management in planning, implementation and monitoring of care, presenting and communicating information

 
 
Delivering a quality service

  • Produce accurate, contemporaneous and complete records of resident consultation, consistent with evidence based practice and the policy and procedures of VMP
  •  Deliver quality care while maintaining all company metrics pertaining to productivity
  • Recognize and work within own competence and professional code of conduct as regulated by the Wisconsin Board of Nursing and American Nurses Association Code of Ethics.
  • Prioritize, organize and manage own workload in a manner that maintains and promotes quality. 
  • Deliver care according to evidence-based care.
  • Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.
  • Initiate and participate in the maintenance of quality systems and processes across the organization and its activities.
  • Utilize the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required.
  • In partnership with other clinical teams, collaborate on improving the quality of health care responding to VMP and other QAPI initiatives as appropriate.
  • Evaluate residents’ response to health care provision and the effectiveness of care. 
  • Support and participate in shared learning across the organization. 
  •  Use a structured framework (e.g. root-cause analysis) to manage, review and identify learning from resident complaints, clinical incidents and near-miss events 
  • Assess the impact of policy implementation on care delivery and communicate those findings to the appropriate team members.

EDUCATION and/or EXPERIENCE
Master’s Degree in Nursing
5+ years’ nursing experience
Valid nurse practitioner license
Ability to work within a team and take direction
Must be familiar with the EHR medical charting system
Strong written and interpersonal communication skills
